How do antibiotics work, and on what?

Antibiotics aim at parts that bacteria have but human cells do not (or that are shaped differently). That is why they can attack only the bacteria. Touch a number and that spot in the picture lights up.

Stop the cell wall from being builtBeta-lactams (penicillins = amoxicillin, cephalosporins = cephalexin, etc.), glycopeptides (vancomycin)When the bacterium divides, it cannot build its wall (peptidoglycan), so the wall bursts and it dies (bactericidal). Human cells have no wall. Works poorly on bacteria that are not multiplying, or that are inside pus or cells. Vancomycin is too big to pass the outer membrane, so it does not work on E. coli.Cephalexin, amoxicillin and vancomycin in this app
Stop the protein factories (ribosomes)Macrolides (clarithromycin, etc.), tetracyclines, aminoglycosidesBacterial ribosomes are shaped differently from human ones (70S in bacteria, 80S in humans). Proteins cannot be made, and most bacteria stop multiplying (bacteriostatic).
Stop the gene (DNA) from being copiedFluoroquinolones (levofloxacin, etc.)They block the bacterial enzyme that untangles DNA, so the bacterium can neither copy it nor divide (bactericidal).
Stop folic acid from being madeTrimethoprim-sulfamethoxazole (ST)Bacteria make their own folic acid (needed to build DNA), but humans get it from food. Used for bladder infections and more.
Break the outer membranePolymyxins (colistin)They punch holes in the outer membrane of gram-negative bacteria. Strong side effects, so a last resort when nothing else works.

They do not work on viruses

Viruses have no wall and no ribosomes; they multiply inside human cells using the human cell’s own machinery. So antibiotics do not work on flu, COVID-19, norovirus or colds (viruses need antiviral drugs).

Bacteria that resist (resistant bacteria)

Bacteria survive by ① having enzymes that break the drug (beta-lactamases, such as ESBL in E. coli), ② changing the shape of the target (MRSA has a different wall-building enzyme), ③ having pumps that push the drug out, or ④ having fewer doors for the drug to get in. Each time a drug is used, the resistant survivors can spread more easily. It is important to use antibiotics only when needed, exactly as a doctor says.

Good bacteria in the gut also decrease

Antibiotics also reduce the helpful bacteria living in the gut. That can cause diarrhea.

Second infection, vaccines

After one fight, some of the T cells and B cells that match that enemy remain as “memory cells”. The second time, the body fights faster and harder. A vaccine is a way to make this memory without getting sick.

Questions and answers

Do viruses just wander around the cells?

Viruses cannot move on their own and cannot multiply outside cells. They are carried by the flow and jiggling of mucus, and when one happens to stick to a cell’s surface, the cell swallows it. Inside, its genes go into the nucleus and use the cell’s machinery to make copies of the virus. In about 6 to 8 hours, new viruses come out of the surface (thousands from one cell). On screen, speech bubbles tell you about ① sticking, ② being swallowed, ③ copying in the nucleus and ④ coming out.

So are the viruses increasing or not?

The “status now” at the top of the screen shows how many are “born” and how many “disappear” per day right now. If more are born, they are increasing. For the first 2 days or so they multiply tens of times every 6 to 8 hours, peaking on day 2 or 3. After that, protected cells increase, killer T cells and antibodies arrive, and the numbers fall. The colored band on the chart (red = increasing, orange = peak, green = decreasing) shows it too.

What happens to infected cells? Do they die?

Yes. A cell that keeps making viruses wears out in about a day. Before that, NK cells or killer T cells may find and destroy it (wrecking the virus factory stops the spread). Dead cells peel off, and macrophages eat up the pieces. Cells underneath divide to fill the gaps, and it heals in 1 to 2 weeks. A cough can linger until the cells with cilia are all back.

Why does a high fever happen?

The virus does not make the heat. Signal substances (interferon, IL-1, IL-6, TNF and others = cytokines) released by infected cells and immune cells travel in the blood to the brain, where the temperature center (hypothalamus) makes a substance called prostaglandin E2 and raises the “target temperature”. To reach the target, the body makes heat by shivering (chills) and narrows the blood vessels in the skin to keep heat in. Fever is thought to help immune cells work and make it harder for viruses to multiply. Fever reducers stop prostaglandin from being made, so the fever goes down, but the viruses do not decrease. Zoom out to “Head” to see the signal reaching the temperature center.

Do macrophages eat viruses?

Yes. But in the nose and throat, the main thing removing viruses is the flow of mucus (and viruses breaking down on their own), and later antibodies (IgA) do a lot. Macrophages are few there, and most of them clean up pieces of dead cells under the lining. Deep in the lungs, macrophages play a much bigger role. What is removing how much right now is shown in the bar of the “status now”.

Where does the mucus go?

Cilia carry the mucus toward the throat, where it is swallowed without you noticing, goes down the esophagus to the stomach, and is broken down by stomach acid. Blowing your nose, sneezing and coughing also push it out. Viruses and antibodies in the mucus are carried along too. Zoom out to “Head” to see the path of the flow.

What are dendritic cells doing with their arms out?

They reach their arms between the lining cells up to the surface and collect pieces of things coming from outside (keeping watch). With a piece in hand, they travel through lymph vessels to a lymph node, where they look for killer T cells and B cells that “match this piece” and alert them. The alerted cells multiply tens of thousands of times and join the fight a few days later. That is why the lymph nodes in your neck swell.

Why is the second infection milder?

Some of the killer T cells and B cells that multiplied the first time stay for years as “memory cells”. Cells that “match that enemy”, which were only a few hundred at first, are now hundreds of times more, and they react quickly to even a few pieces. Cells that keep making antibodies remain too. So the second time, the wait before they start working shrinks from 5–7 days to 2–3 days, and the infection is often cleared before it grows. But flu and COVID-19 slowly change their surface shapes, so old antibodies stick less well and people catch them again. Try it with the “Second infection, vaccines” buttons.

Why do vaccines work?

They show only pieces of the germ (or the blueprint for a piece) in advance, so memory cells and antibodies are made without getting sick. When the real thing comes, the body can fight fast, just like a “second time”. A sore arm or fever after a shot is a sign the immune system is reacting. Vaccines not only help prevent infection, they also make it less likely to become serious.

How are bacteria different from viruses?

Bacteria (like Staphylococcus aureus) take in nutrients by themselves and multiply by splitting in two outside cells. That is why antibiotics that stop the cell wall from being built work on them. Viruses can only multiply inside cells, so antibiotics do not work; antiviral drugs block other steps of how viruses multiply.

About this app

Behind the scenes, the changing numbers of germs, infected cells, immune cells and antibodies are calculated with the kind of equations used in research (differential equations). The micro view and the whole-body window move to match those numbers.

KnownHow things work (what does what), and rough timing (6 to 8 hours until new viruses come out, killer T cells around day 5 to 7, etc.).

EstimateNumbers, body temperature and amount of damage. These are rough values from the model and differ a lot from person to person.

DisplayThe number of dots on screen is not the real number (it reaches hundreds of millions, so we draw on a “log” scale that adds a dozen or so dots for every 10×). Viruses and antibodies are drawn much larger than they really are.

What we simplified: for every germ, the infected area is treated as one place (nose and throat, small intestine, around a wound, part of a lung, bladder). Spread to the lungs or kidneys is only shown as “may spread when damage or bacteria are high”. Neutrophils also gather in flu and COVID-19, but they are not included in the screen or the model. Mucus really flows much faster (a few mm a minute) but is drawn slowly. Medicines are treated as having a steady strength, leaving out the ups and downs between doses. Vaccines are treated as “giving pieces for a few days”, and the differences between types (inactivated, mRNA, conjugate) are shown only as strength. For the days between first and second infections, the model runs with no germs to calculate how antibodies and memory cells fade. Other illnesses and fine differences by age are not included.

The numbers are rough values tuned to match well-known courses of illness (days of fever, days to recover, days shortened by medicine).
